No soy un experto en Perl, pero lo que sí sé es que GMail es compatible con IMAP y POP3, 2 protocolos que son completamente estándar y le permiten hacer precisamente eso.

Dentro de gmail, puede filtrar en "has:attachment", usarlo para identificar los mensajes que debe recibir al probar. Tenga en cuenta que esto parece dar tanto mensajes con archivos adjuntos (se muestra el icono del clip), como imágenes adjuntas en línea (no se muestra el clip).
